Thank you for your interest in our Aesthetics Membership Program. Please review the following terms carefully before enrolling.
• Membership is $99 per month, billed automatically to the card on file.
• Each monthly payment is added as a credit ("bank") that can be used toward aesthetics services.
• Enjoy 15% off all services when using your membership credits.
• Payments are non-refundable.
• A minimum commitment of 3 months is required.
• After 3 months, membership may be canceled at any time with written notice.
• Upon cancellation, you have 90 days to use any remaining credits. After 90 days, all unused credits expire and cannot be refunded or transferred.
• First appointment: A $99 deposit is required and will be applied toward services.
• Follow-up visits: A valid credit card must remain on file for membership billing and fees.
• No-shows or cancellations within 48 hours of the appointment will result in a $99 fee, charged to the card on file.
• Credits may be applied toward eligible aesthetics services only (not transferable to products, medical visits, or other patients).
• Membership does not guarantee appointment availability; scheduling is subject to clinic availability.
• The clinic reserves the right to modify or discontinue the membership program with advance notice.
Important: Please read all terms carefully before enrolling. This agreement becomes effective upon enrollment and payment of the first membership fee.
